I find correcting errors of omission to be the most beneficial aspect of MedHx. A patient was admitted to my treatment team and a home medication was left off because the admitting providers did not know the patient was supposed to be taking it (not in the patient’s belongings). The initial med reconciliation (without MedHx) was rushed and this medication was left out of the admission orders, which could have potentially caused the patient to experience withdrawal symptoms. I ran a MedHx search before rounds started the next day and was able to identify the missing medication. I made sure the treatment team restarted it to avoid further patient discomfort.
